Through case management, individuals achieve successes such as acquiring their driver’s license, purchasing their first car, and moving into their first apartment. Others receive the support they needed to take care of their children as single parents, enroll in their first college-course, or made a smooth transition to a new town where they were connected with needed services in their new community.

Trained and compassionate case management staff provide:

• Case Management

• Respite Services

• Adult Daily Living Skills Training

• Safety Skills Training

• Facilitated Access to government assistance

• Community Inclusion

• Budgeting Assistance

• Housing Coordination

• Health/Medical Care Coordination

• Individualized Supports Identified by the Individual & Team

"My son has been a client for three years. He's 46. I've never seen him so happy - he has friends, attends social events, and the program even helped him find a part-time job. His life he has a purpose now."
- Parent of Developmental Services Client
