Families and Communities Together (FCT) helps families to identify and use their natural and community supports as well as health and social services to create holistic support system to provide care and treatment for children and meet the needs of families.
Through formal health and social services and natural and community supports such as friends, family, community members, churches, and schools, families are able to form a support and treatment network that meets all of the needs of the child and family. Culturally competent staff and care coordinators work with families to create the support network and to design a comprehensive plan for the child’s future.
Accessing all of the supports and resources available to you can be overwhelming. FCT can help your child and your family.

Staff and care coordinators work with each family in a individualized and culturally competent manner to meet the needs of each family.
FCT is a new collaboration of the Willis center, Community Healthlink, and LUK,Inc.
